Paperwork

One packet, sent to you before you come in. Fill it in at your own kitchen table, not on a clipboard in a waiting room with a pen on a string.

Your first visit should start with your story

Most people arrive at a clinic and lose the first fifteen minutes to a clipboard. You sit down, you write your birthday for the fourth time, and the appointment you are paying for is already shorter.

We do it the other way around. Once your visit is booked, we send your intake packet to you ahead of time. You fill it in at home, at whatever hour actually suits you.

There is nothing to hunt for and download here. We send yours directly, and if anything on it is unclear, call (216) 577-1933 and someone will go through it with you.

What to bring

  • Your photo ID.
  • Your referral or prescription, if a physician gave you one. Ohio does not require it, so no referral is not a problem.
  • Any imaging reports you have: X-ray, MRI, surgical notes. The report matters more than the disc.
  • A list of the medications you take.
  • Clothes you can actually move in, and the shoes you wear most days rather than the good pair. We want to see how you really walk.

What we ask you, and why

The packet covers your history, consent, and privacy. That part is routine.

The questions that matter are the other ones. Where it hurts. When it started. What makes it worse by Thursday. What you have already tried and what happened when you tried it.

And the one people hesitate over: what do you want to get back to. Carrying a toddler on one hip without bracing. Sleeping on that side again. Standing through a long morning without counting the minutes. Say the real answer. That answer is what your plan gets built toward.

What happens with it

Your forms are read before you arrive, not after you leave. So your first appointment opens with a conversation and a cause-first evaluation instead of a transcription exercise.

You are one-on-one with a licensed physical therapist from the start, and you leave knowing what is wrong, what is driving it, and roughly how long it takes to fix.

Your information stays with the people treating you. Our privacy policy says exactly how we handle it.

You fill in everybody’s forms but your own

The school ones. Your mother’s pharmacy ones. The paperwork nobody else in the house will touch. You are the family administrator and you never asked for the job.

So it makes complete sense that your back has been on the list for eleven months and never at the top of it. That is not avoidance. That is a person with too many other people to look after.

But the list does not clear itself, and the thing you are carrying gets heavier the longer you carry it in silence. One packet, one hour, one honest answer. That is the whole ask.

Ready when you are: see what we treat or get in touch.
