Weeks 1–2
Pain
Calm it down.
Hands-on work and better positioning settle the irritated disc or joint, so sitting, standing and sleeping stop being a negotiation.
What We Treat
You have already rearranged the day around it. How you sit. How you lift. Which chair you pick. A cause-first evaluation finds what is actually driving the pain, so you can stop guessing and start fixing.

Two or more? Your back is telling you something. Keep reading.
Why It Happens
A low back almost never hurts for one reason. It is usually four things stacked on top of each other. A disc that is irritated enough to send pain down a leg. Spinal joints that stiffened up and stopped taking their share of the load. Deep stabilizing muscles that switched off quietly after the last flare-up. And the way you sit, bend and lift, which reloads the same spot before it ever finishes healing.
Sciatica belongs in that same story. It is not its own disease. It is a nerve being crowded somewhere along its path, and the useful question is where, and by what. That is what a cause-first evaluation is for. Rest, ice and a printed stretch sheet all aim at the sore spot. Find what is actually driving it and the plan writes itself.
The Honest Truth
You are the one who carries everything for everyone else. Which is exactly why you are the last one to get looked at, and exactly why you are reading this.
How We Fix It
Every plan has an endpoint. Here is what the four phases look like for a low back.
Weeks 1–2
Pain
Hands-on work and better positioning settle the irritated disc or joint, so sitting, standing and sleeping stop being a negotiation.
Weeks 2–6
Prime
We free up the stiff spinal segments, and the tight hips that have been borrowing movement your back should be doing itself.
Weeks 6–10
Perform
Deep core and glute strength gets loaded on purpose, until your spine has real support underneath it instead of a brace.
Weeks 10+
Prevent
Lifting, bending and twisting get retrained under real weight, so the movement that set this off stops being the thing you fear.
Not only is Dr Joseph Moskowitz a wonderful Physical Therapist, he is very knowledgeable, caring, and motivating! He knows just how to work on, or manipulate, each of my aches and pains, to help improve my strength, core, and posture! I would highly recommend In Motion Physical Therapy!

Questions, Answered
Usually not, and waiting on one costs you weeks. Most low back pain settles without imaging. Scans of people with no pain at all routinely show bulging discs and wear and tear, so a picture rarely names the cause by itself. We start with a hands-on, cause-first evaluation and work out what is actually driving your symptoms. If imaging turns out to be genuinely necessary, we will help you get it.
Done right, no. Full rest quiets a back for a day, then leaves it stiffer and weaker, which is what sets up the next flare-up. The skill is picking the movement your back tolerates today, and adding load only as it earns the load. That is what one-on-one with a licensed physical therapist is for, every visit.
You get a plan with an endpoint, not an open ended string of appointments. Your back plan moves through four phases: Pain, Prime, Perform, Prevent. At every visit you know which phase you are in and what is left before you are done. How long that takes depends on how long you have been guarding it, and we give you a straight answer after the evaluation.
